You should include the subject code and quiz number, information on what happened, any screenshots of error messages (if applicable), and which internet browser you are using.
The details of the situation will be reviewed and a determination of your case made, either by the Online Helpdesk team or by a Senior Academic for your subject. If approved they will re-enable your quiz attempt and notify you of the time and dates.
